Not having to change batteries = unrealistic. Sanity Pills = completely realistic.

Sometimes I really wonder if people actually think these kind of things through.

No. All batteries would do is:

1. Limit the time spent using the flashlight for no real reason.

2. Force us to stop investigating to put in new batteries every few minutes.

3. Make us just quit once the batteries ran out.

There's, literally, 0 reason to limit the base flashlights via batteries. It does not add anything as a mechanic or to the gameplay loop itself.

Running out of batteries for your EMF and not being able to test that evidence type anymore would suck.

Setting aside the fact that cell phones would have replaced a number of the items in the game, instead of batteries, maybe a crank charged flashlight. It would meet the desire to pay attention to your light source and be more "realistic" in that you need to recharge it every few minutes or so. Charging it would also mean making a bit of noise so there is the potential for the ghost to hear you but your sanity goes down while in the dark if you don't charge it.

Sounds boring to me. The idea of having to run to charge or get more seems more time consuming and time draining than anything else.

Batteries are a terrible idea because they would repeatedly interrupt the flow of the game, and would be annoying at best, and useless at worst.

ok i can take Lights needing batteries as long as it frees up 1 hand so i can carry 3 items and have the shoulder light or a head mounted one

if the ghost can drain battery life. maybe. But having to run back to the van for extra battery but someone already took them all can be very annoying. Meaning you can't see ♥♥♥♥ now because your flashlight is dead.

tbh the matches hardly ever last longer then 30 minutes, on small maps its never really over 20 mins
a normal battery in a turned on flashlight probably works for 2-3h
I could see an emf using so little battery it holds for 24h
I have 1 camcorder and 1 go pro and they both have at least 2h of battery when filming

so I don´t really know if it would make sense unless we say the ghost actively drains battery during a hunt or ghost event which would make it possible to maybe find the rajiu earlier as it drains more battery or so-so

Batteries are a terrible idea in basically every horror game that has them. No, it's not realistic. You put batteries in a flashlight and they last for quite awhile, not 5 minutes.
